• Clog share (30%). How much of your line the repeat questions honestly consume — most owners have never estimated it, which is itself scored.
• Answer-call burden (25%). How long a typical insurance answer takes — the two-minute lookup versus the ten-minute plan spelunk with callback.
• Publishing habit (25% — the persistence axis). Where researched answers GO: nowhere, a head, a cheat sheet, or the public page that retires the question forever.
• Deflection surfaces (20%). Whether current answers exist anywhere a patient can find without calling — website, text-back, Google profile — and whether they can be trusted.

Self-reported first, measured second

The free score is what you tell us, labeled as that. The $500 discovery runs the two streams: timed calls asking your actual commonest questions (durations, holds, callbacks logged) and timed new-patient attempts into the same peak windows (get-through rates, wait times) — per your written permission, every event timestamped at the source. Plus a findability audit: what a patient searching your answers actually finds, including what the AI assistants say about you. You get the measured clog beside your self-reported one.

What we don't do

• Test calls are brief and ordinary — the kind your desk already fields hourly. Every probe is identified to you after the window closes.
• No probes on hours you exclude. Permission is written and explicit.
• No padded findings. "Your line is clear and your answers are findable" is a deliverable we're glad to hand you, in writing.
